Adam's farm becomes a site of tragedy when the Ghoul extracts information about Lee Moldaver's location by threatening Adam's family, ultimately killing his son Tommy after he reaches for his gun. In a flashback, it is revealed that Cooper, before the Great War, was recruited by Moldaver (then called Miss Williams) to spy on his wife Barb and steal information about a cold fusion project that Vault-Tec had suppressed. Meanwhile, Lucy is exiled from Vault 4 for trespassing, but Maximus helps her escape to the surface where they meet Thaddeus, who discovers that a healing serum is turning him into a ghoul, forcing him to flee from the approaching Brotherhood of Steel. Norm discovers a conspiracy involving the overseers when he learns that Betty is actually communicating with someone in the mysterious Vault 31, leading him to uncover a shocking secret hidden within that vault.
