In the post-apocalyptic wasteland, multiple factions pursue Wilzig's severed head for unknown reasons: Lucy MacLean, a naive Vault dweller, carries it while being hunted by the Ghoul, a mysterious scarred survivor; Maximus, a Brotherhood of Steel soldier impersonating a dead knight, and his reluctant squire Thaddeus track them all down; and the residents of Vault 33 face a crisis when their water chip breaks, forcing them to consider dangerous options. Through flashbacks, it's revealed that Cooper Howard, the Ghoul's pre-war identity, was a Hollywood actor who unknowingly modeled for the iconic Vault Boy mascot while his wife worked for Vault-Tec, the company that would later vault him away during the nuclear apocalypse. As all parties converge on the gulper creature that swallowed the head, the stage is set for a dangerous collision between the wasteland's survivors, each with their own stakes in retrieving this mysterious prize.
